JISEC-CCM-02-A Forms to be downloaded (for Certification Application)

The following are the forms needed for the certification application. Make sure to follow the notes and instructions when submitting application forms.

• Forms may be changed for application and administrative convenience. Make sure to use the latest versions of the forms when needed.
• For completing the documents, refer to the Requirements and Guidance on IT Security Certification (CCM-02, CCM-02-A).
• The application or request forms marked with Word can be submitted via email. Forms should be sent by the person in charge of the application (described in Form 1-1) and data should be encrypted as necessary.
• In the case that the submitted documents are not complete, the documents will be returned to the applicant because insufficient documents cannot be accepted.
  If there are any unclear items about submitting application, feel free to contact with the Certification Body in advance.
